Please read through & then complete the form BELOW. This can be completed on a laptop, desktop computer, and/or mobile device.
All fields must be completed, including insurance information.
We are in-network providers for Blue Cross/Blue Shield, UPMC HealthPlan, and Medical Assistance (CCBH and Carelon). We are NOT in-network with UnitedHealthcare, Aetna, Cigna, WPEE, or Tricare.
We must be in-network with BOTH your primary insurance AND secondary insurance in order to bill Medical Assistance.
You will receive an email within one to two weeks of receiving your request form with your scheduled appointment time.
To avoid our emails going to your spam folder, it can be helpful to add our intake address (intake@cpnpgh.com) to your contacts.

Three weeks prior to your scheduled appointment, you will receive a confirmation call or email. You MUST confirm your appointment, or it may be canceled.
Once your appointment is confirmed, you will receive consent, releases of information, and history forms VIA EMAIL that must be completed prior to your appointment.

Evaluation Intake Form
We are Expanding! As our new providers wait for insurance credentialing, we have a limited number of Private Pay Neuropsychological Evaluation appointments available with less than a month's wait. Saturday Private Pay appointments are also available for both Early Childhood Evaluations (under age 4) and Neuropsychological Evaluations (ages 4-6 only). Please see our Fee Schedule for pricing and indicate in any "Other" Section on your Appointment Request form that you would like a Private Pay appointment. Unfortunately, we are unable to see children with Medical Assistance for any Private Pay Services.
Please be aware that we are currently scheduling neuropsychological evaluations 6-12 months out, depending on your child’s specific needs and insurance. Every child eligible for evaluation will be offered an appointment.
We ask that you only submit this form if you have decided that you would like to be scheduled for an evaluation despite our wait.
Our intake emails frequently get filtered as spam or promotions. Please be aware that we do most of our scheduling via email. Missed emails can lead to delays in scheduling. To avoid missing our communications, please add our email address (intake@cpnpgh.com) to your contacts.
