To Schedule a NEW appointment

• Please complete the New Appointment Request Form and email it to intake@cpnpgh.com.

• All fields must be completed, including insurance information.

• You will receive a call within 72 hours with your scheduled appointment time. 

Prior to your scheduled appointment

• Please review the Client Information for Assessments and our Fees for Services.

• Complete the Health History Form and either email it to your provider or print it and bring it with you to your appointment.
